incentives. The greatest challenge is enabling seamless care coordination because health care is managed by 
so many different stakeholders throughout the value cycle with so many steps of the care transition. Increase 
trust between health care facilities and scalable and distributed data exchange because interoperability is 
essential for a successful digital health care program.

for all known patient health card exchanges. They can be further upgraded with FHIR approval, which can also 
be represented in smart contracts, and offer full flexibility to members to change approval levels if necessary. 
Some of the old issues in the industry, such as the phasing out of Medicaid or continuity of coverage as 
beneficiaries keep coming and going due to changes in eligibility, can mostly be resolved by establishing a 
Smart Health Profile.  
 
In the long term, artificial intelligence could influence the creation of an entirely new healthcare funding 
model behind the medical blockchain. Although blockchain is still in the making, it has developed brand 
awareness because of its focus on digital health and more federal healthcare, and is open to the introduction 
of new models of care and coordination. The blockchain is here to stay!
