consultations for  seniors 60 and up
An affordable healthcard for senior citizens 60 years old and up. It 
covers  unlimited outpatient consultations and unlimited availment 
of up to 255 lab  and diagnostic procedures at Maxicare Primary 
Care Centers and My Health  Clinics, annual ER coverage of P20,000 
at all Maxicare-affiliated hospitals, FREE  annual select dental 
services at Metro Dental, and access to our 24/7 Tele  Consult 
Service. Pre-existing conditions are covered, and membership is 
valid  for one year.
Offers unlimited outpatient consultations, lab tests, and 
diagnostics, and an  annual ER coverage of P20,000 for senior 
citizens 60 years old and above.

2. Maxicare PRIMA Silver - Unlimited lab tests, diagnostics, and 
consultations for  individuals 0 to 59 years old
An affordable prepaid health card for individuals 0 to 59 years old. It 
covers  unlimited outpatient consultations and unlimited lab and 
diagnostic  procedures at Maxicare Primary Care Centers, FREE 
annual select dental  services at Metro Dental, and access to the 
Maxicare Telemedicine 24 x 7  Voice Service. Pre-existing conditions 
are covered, and membership is valid for  one year.
Offers unlimited outpatient consultations, lab tests, and 
diagnostics for  individuals 0 to 59 years old.
How to Activate Your Maxicare Healthcard?
After purchase, you need to register your card before you can use it.
Via ONLINE -
1. Visit the Online Card Registration Page
2. Fill in the Reference Number, which you’ll find in your purchase  
receipt email
3. Provide the required personal details and click “Submit”
4. Once successfully registered, you will receive your PRIMA Gold  
electronic health card (e-Card) via email along with your policy  
and card number via SMS
5. You may avail of your unlimited consultations and laboratory as  
soon as you receive e-Card
6. Your Emergency Care benefit will be activated 15 days after  
successful registration
7. Your Maxicare PRIMA Gold membership card will be delivered  
to your nominated address 15 working days after your  
registration
