events. This support is great to help you feel connected to 
others and the broader community and to prevent social 
isolation. Our staff can support you to attend any activity that 
you chose whether it be one-to-one or in a group setting.

Support Co-Ordinators at Journey Health Solutions, will assist 
you to work towards your identified goals in your NDIS plan. 
This will involve navigating the NDIS service system, making 
informed decisions and obtaining the resources and support
