We report a rare case of an anomalous origin of the left circumflex artery (LCX) from the proximal right coronary artery (RCA), along with an independent origin of the left anterior descending artery (LAD) from the right coronary cusp. Recognition of this coronary anomaly was pivotal in the preoperative assessment of a patient undergoing surgical aortic valve replacement (SAVR) for severe aortic stenosis.
