Universal Personalised Care
• Universal Personalised Care is the delivery plan for personalised care
• It is about implementing the model of personalised care to 2.5m people by 2023/4 and then
double again before 2028/29.
• Personalised care means people have choice and control over the way their care is planned, and
delivered, based on what matters to them and their individual strengths, needs and preferences.
• This happens within a system that supports people to stay well for longer and makes the most of
their expertise, capacity and potential.
• The components are:
• 1. Shared decision making
• 2. Personalised care and support planning
• 3. Enabling choice, including legal rights to choice
• 4. Social prescribing and community-based support
• 5. Supported self-management
• 6. Personal health budgets and integrated personal budgets.

Aim of PHBs/Integrated Budgets
• To give people greater choice, flexibility and
control over the healthcare and support they
receive
• An opportunity for people to work in equal
partnership with the NHS and local
authorities about how their health and
wellbeing needs can best be met
• Personal health budgets/integrated personal
budgets are not about new money, but about
using resources differently

• Viability of a legal right to IPBs including wider funding
streams
• Policy and legislative implications
• Opportunities and challenges to the system
• Working with CCGs, local authorities and others to
gather their views
• What are the unintended consequences?

• Reporting to Treasury, DHSC, DWP and MHCLG

In an ideal world…
A truly integrated personal budget would involve:
• One assessment
• One indicative budget
• One set of outcomes
• One person centred care and support plan
• One direct payment
• One process for review and one audit
But there are challenges before we get to an ideal
world…

Solutions and work arounds
• Based on good relationships between CCGs and LAs
• Clear communication, personalised care and shared
decision making with the budget holder at the centre
• Integrated assessments are key – identifying health
and social outcomes as well as the costings
• Leading to joint commissioning arrangements / re-
charging between organisations to facilitate a single
direct payment
