c a r e  t o  c h ro n i c  d i s e a s e  m a n a g e m e n t .  T h e i r  
a b i l i t y  t o  p ro v i d e  c o m p re h e n s i v e  c a r e  
m e a n s  t h a t  p a t i e n t s  c a n  r e c e i v e  m o s t  o f  
t h e i r  h e a l t h c a re  n e e d s  w i t h i n  t h e i r
