A wide range of services are provided to senior citizens as determined necessary by a healthcare screening process as well as their choice of residence. This program is open to people in Connecticut aged 65 and older who are at risk of being taken to a nursing home. There are 4 tiers of eligibility that are relevant to individuals aged 65 years and above. Two of these tiers are state funded while the other two are reimbursed via Medicaid. With that said, anyone that meets or has a greater need than described below qualifies for the program. Things To Know About The CT Homecare Program For Elders Before anyone can be enrolled into the CT homecare program for elders, they should have a functional need. The candidate should have a critical need for assistance with at least one or two activities of daily living, such as eating, personal hygiene, dressing, transferring as well as the ability to maintain continence and use a toilet. The person who wants to be enrolled into the CT homecare program for elders should also have a financial need. There are two sets of financial eligibility criteria, namely stated funded and Medicaid waiver. One of these two must be met for the individual to receive services under this program.
