CAUSES OF HEARING LOSS: OVERVIEW
Age-related hearing loss (presbycusis) refers to gradual changes
in hearing ability that occur over time. In contrast, hearing loss
can also result from a range of other causes, including medical
conditions, structural issues, or environmental exposure.
It is important to recognise that hearing loss varies depending on
its underlying cause. Some forms are temporary or medically
treatable, while others are permanent and managed through
rehabilitation strategies such as hearing aids.
WHAT CAUSES AGE-RELATED HEARING LOSS?
Age-related hearing loss develops from a combination of long-
term changes in the auditory system. These may include:
· Gradual loss or damage to inner ear hair cells
· Changes in auditory nerve pathways
· Reduced blood supply to the inner ear
· Cumulative lifetime exposure to sound
· Structural changes within the ear
Certain factors may also contribute, including:
· Long-term exposure to loud noise

affects both ears.
Common signs include:
· Difficulty understanding speech, especially in group
conversations
· Reduced clarity of speech rather than just volume
· Increased difficulty hearing in noisy environments
· Frequently asking others to repeat themselves
· Turning up the volume on devices
· Listening fatigue in social settings
· Tinnitus (ringing or buzzing in the ears) may occur, but is not
a defining symptom.
OTHER CAUSES OF HEARING LOSS
Hearing loss can also result from a variety of underlying causes.
These broadly include issues affecting sound transmission, inner
ear function, or both.
Conductive Causes (Outer or Middle Ear)
These affect how sound travels through the ear and may include:
· Earwax blockage
· Middle ear infections
· Fluid in the middle ear
· Problems with the eardrum or middle ear bones
These conditions are often medically managed and, in many
cases, treatable.
Sensorineural Causes (Inner Ear or Nerve)

Other causes of hearing loss may occur either gradually or
suddenly, depending on the underlying condition. These causes
can include infections, ear blockages, structural abnormalities, or
other medical conditions. Symptoms may extend beyond
reduced hearing to include ear pain, a feeling of pressure,
dizziness, or sudden hearing loss. Management typically depends
on the specific cause and may involve medical treatment,
regular monitoring, or hearing rehabilitation to restore or
improve hearing function.
WHEN TO SEEK MEDICAL ATTENTION
Sudden changes in hearing should always be treated as urgent.
If you experience:
· Sudden hearing loss
· Hearing loss in one ear
· Hearing changes with dizziness or pain
You should seek medical attention promptly, ideally through a
GP within 24–72 hours.
THE ROLE OF AUDIOLOGISTS AND GPS
A GP is typically the first point of contact when a medical cause
is suspected.
An audiologist can:
• Conduct comprehensive diagnostic hearing assessments
• Identify the type and degree of hearing loss
• Guide referrals to a GP or specialist when needed

provide enough detail. A full diagnostic assessment is
recommended for accurate diagnosis.
