Unity Care serves foster youth across a number of California counties, including El Dorado, Santa Clara, San Francisco, Placer, San Mateo, Monterey, Alameda and Nevada counties. Founded in 1993 by André Chapman, the organization was created to provide support, stable housing, caring connections, and life skills which assist youth as they transition out of the system and begin navigating the challenges of independent living. Unity Care Group serves youth ages 16 to 21.
